Work Together NC will be presenting on the Community Transition Plan, focusing on the supports and resources that will help our teens/young adults transition well into community-based life.
(From their website:) Using a collective impact approach to facilitate cross sector
collaboration and systems change, we aim to… 1) reduce complexity and increase easier access to services supporting the transition to adulthood, starting in the Triangle/Triad area of NC and eventually scaled statewide 2) support people with ID/DD to access and advance in meaningful work and/or continued education/training.
